Job Description:

Maine Children’s Home has been providing social work, counseling, training, guidance, and support to children, youth and families since 1899. These services and supports have shifted and evolved over the course of MCH history but the focus has and continues to be on the most vulnerable in our communities. We are looking for a Targeted Case Manager to join the passionate, committed staff at MCH in meeting needs of children and families in our community. The Targeted Case Manager is responsible for providing Maine Care authorized advocacy and service coordination services for children, adolescents, and their families in their home, at school and in their local community. The Targeted Case Manager also assists with navigating the policies and procedures of local and state entities.

Core Duties

  • Assess and evaluate client needs, completing all necessary paperwork as required by state, insurance and MCH regulations and policies.
  • Be aware of and understand all local client resources, staying current on changes.
  • Meet with clients and providers on a regular basis to maintain and coordinate services.
  • Make referrals to other agencies and services as appropriate.
  • Develop individualized service plans for all clients and update them on a quarterly basis or sooner if needed
  • Manage transition towards termination or referral to other services.
  • Respond appropriately to emergencies, crises and unexpected situations.
  • Be aware of and adhere to the policies and procedures of Maine Children’s Home and the counseling center.
  • Participate in regular clinical supervision and seek additional supervision as indicated.
  • Using the electronic records system, complete documentation within established timelines and professional standards per MCH guidelines, licensing/accreditation requirements, and those required by Maine Care.
  • Provide clients with information and education regarding their mental health needs, their rights, relevant resources, and means of access.
  • Assure confidentiality and rights of clients.
  • Serve as an advocate for clients.
  • Participate in assigned quality assurance activities.
  • Meet or exceed the established billable expectations for the position.
  • Participate in community activities as schedules permit.
  • Participate with supervisor in creating an individual professional development plan that ensures compliance with licensing and supports the programs strategic goals.
  • Respond to requests from external agencies when appropriate and in a timely manner.

Education/Experience:

The Targeted Case Manager shall have a minimum of a Bachelor’s Degree in Social Work, Psychology or a related degree from an accredited program with not less than two (2) years experience working with children and families. The Targeted Case Manager will have strong organizational ability, be detail oriented and have experience working with Microsoft programs such as Word and Excel, and with electronic records. They must also possess a valid driver’s license with no criminal violations and be able to pass background checks. Local travel to client locations is required.
